Depression is an important mental health disorder which affects many people globally. Inpatients are among the groups of people who are at high risk of experiencing depressive symptoms. The primary aim of this review was to assessment the manner in which exercise impact the management and prevention of inpatients with depression. In order to achieve this goal, literature review was conducted on the existing literature. The literature search process was conducted on different databases such as PubMed, CINAHL, Psych Info, ScienceDirect, NELSON and Elsevier. A total of 12 studies were selected and included in this review having met all the eligibility criteria. Thematic analysis of evidence from these studies led to the generation of three themes which include exercise programmes to manage depression, psychosocial and biological mechanisms around exercise, and exercise’s role in depression prevention and treatment. The reported results showed that exercise programme has positive impacts on the depressive symptoms among inpatients.

Objectives and Topics
This critical literature review aims to investigate the role of physical exercise in the prevention and management of depression among inpatients, evaluating its efficacy as a complementary therapeutic intervention.
- The impact of exercise programs on managing depressive symptoms.
- Psychosocial and biological mechanisms linking exercise to mental health.
- The role of varied exercise modalities (aerobics and resistance training) on patient outcomes.
- Clinical implications for nursing practice and patient self-efficacy.
Excerpt from the Book
Psychosocial and Biological Mechanisms Around Exercise
The second theme was developed using evidence from the studies included in this review about the psychosocial and biological mechanisms around exercise and their impacts on depressive symptoms among the inpatients. Evidence from the studies by Godino et al. (2014) and Smith et al. (2019) shows that exercise activates the peroxisome proliferator-activated receptor γ activating factor 1α (PGC-1α) gene present in the skeletal muscle which leads to the modulation of the Kynurenine (KYN) metabolism and improves ability of an individual to be resilient to stress-induced depression. Similar arguments are presented in the study by Dickinson et al. (2023) which reports that the mechanism by which the expression of exercise-induced skeletal muscle PGC-1α leads to the improvement of depressive behaviours is primarily through the regulation of KYN metabolism, expression of the central brain-derived neurotrophic factor (BDNF) and peripheral inflammatory factors, hence having indirect antidepressant effects.
With reference to the arguments by Godino et al. (2014), endurance exercise among the inpatients facilitate the creation of a balance in the skeletal KYN metabolism via the PGC-1α pathway, leading to the alleviation of the neurotoxic effects of KYN, and exertion of the antidepressant effects. Consistent evidence has been reported in the study by Smith et al. (2019) which establishes that aerobic exercise has positive impacts on the skeletal muscle PGC-1α and KAT1-4 expression among depressed individuals. Therefore, it is justifiable to note that exercise leads to the activation of skeletal muscle PGC-1α which plays an important role in the regulation of peripheral KYN metabolism, reduction in the peripheral inflammatory response as well as secretion of neuroprotective factors, hence exerting antidepressant effects among the inpatients.

Frequently Asked Questions
What is the primary focus of this research?
The work investigates the role of physical exercise as a therapeutic intervention for preventing and treating depression among inpatient populations.
What are the central thematic areas?
The study centers on exercise programs for depression management, the psychosocial and biological mechanisms behind exercise, and the role of exercise in clinical treatment.
What is the main research question?
The central question is: How does exercise impact the management and prevention of inpatients with depression?
Which research methods were employed?
The author conducted a critical literature review, screening 12 selected studies from databases such as PubMed, CINAHL, and ScienceDirect using PRISMA guidelines.
What does the main content cover?
It covers empirical evidence on exercise-induced biological changes, such as PGC-1 alpha activation, and provides practical recommendations for nursing practitioners.
What are the characterizing keywords?
The work is characterized by terms such as depression, exercise, inpatients, PGC-1 alpha, and holistic mental health care.
How does exercise impact KYN metabolism?
According to the text, exercise leads to the activation of skeletal muscle PGC-1 alpha, which helps regulate peripheral KYN metabolism and reduces inflammatory responses.
Why is the FITT parameter important mentioned in this review?
The review notes that FITT parameters (frequency, intensity, time, type) are often inconsistently reported, creating challenges for reproducing results and establishing optimal dosages.
What role do nurses play according to this paper?
Nurses are responsible for educating patients and their families about the benefits of exercise and implementing augmented exercise programs to reduce depressive symptoms.
