Mechanism of Pathophysiology

This disease is caused by atherosclerosis, which seriously affects the usual structure and function of the coronary vessels. The first step in the disease process is endothelial dysfunction—the smooth inner layer protecting the arteries becomes damaged by risk factors, including oxidized low-density lipoproteins, high blood pressure, diabetes, smoking, and inflammation. As a result of endothelial injury, the artery's walls become more porous, and lipoproteins and immune cells can move into the wall and build up underneath the endothelium, triggering a chain of inflammatory effects. Monocytes migrate into the vessel wall and differentiate into macrophages, which engulf oxidized lipids to become foam cells, forming the earliest lesion known as a fatty streak (Dhakal & Pokharel, 2024). With time, smooth muscle moves from the media into the intima, multiplies, and produces collagen and elastin, adding to plaque and making the artery thicker. As plaque deposits get larger in the artery, they gradually restrict the artery so that when demand for oxygen to the heart rises, flow-limiting stenosis appears, making angina pectoris more likely to happen. When thin-capped plaques filled with large amounts of lipid burst, the blood contacts the plaque contents. It promotes acute thrombosis, which can shut off the coronary artery and result in a heart attack and serious complications, including death.
