For instance, the recent fiasco of the Boeing 737 MAX causing the loss of 346 souls is more than a technology failure. There were indications of real deep problems that have their roots in the organisation leadership (Cusumano, 2021).

Indeed, the US House Transportation and Infrastructure Committee stated that: “cost, schedule, and production pressures undermined the safety of the 737 MAX” (The US House Transportation and Infrastructure Committee, 2020).
